Ukrainians who currently reside abroad but want to refuse social assistance that they received in their homeland must send an application by e-mail or other means of postal communication to the appropriate social security department in Ukraine. Contacts of social security offices can be found on the official websites of district state administrations and city executive bodies.
The following information must be provided in the application:
- Full name of the citizen in whose name the social assistance is registered;
- Address of the registered place of residence;
- Telephone number;
- Name of the social protection agency providing the assistance;
- Type of social assistance;
- E-mail address.
If a citizen needs confirmation of the termination of payments, it should be indicated in the application. In this case, the social assistance body will send a scanned copy of the document confirming the refusal of benefits by e-mail.
