For visa-free entry to the Czech Republic you need a valid biometric passport and get a health insurance policy for the entire period of stay in the country.
Citizens of Ukraine who have been fully vaccinated with one of the EU-approved vaccines listed below can enter the Czech Republic for any purpose without the need for any additional quarantine measures (without the need for testing or self-isolation). Completely vaccinated are persons (must be at least 14 days after the second dose, in the case of a two-stage vaccine) of one of the following vaccines:
- AstraZeneca (Vaxzevria)
- Modern (Spikevax)
- Pfizer BioNTech (Comirnaty)
- Johnson & Johnson (Janssen)
- Covishield
! To obtain an entry permit as a fully vaccinated person, you will need to provide a digital COVID certificate obtained on the Diia app or printed from the Diia portal.
Entry for non-vaccinated Ukrainian citizens to the Czech Republic is prohibited, with the exception of those who have a residence permit in the Czech Republic or a visa issued by the authorities of the Czech Republic. Citizens of Ukraine who have a residence permit in another EU country - an appropriate plastic card (ATTENTION! A long-term visa is not considered such a permit), are allowed to enter if there is confirmation of the purpose of the trip.
Entry in exceptional humanitarian situations (carrying out planned medical examinations or operations, carrying out court decisions, participating in a court hearing, caring for a non-self-sufficient family member, caring for a child, etc.) is allowed if there are supporting documents issued by Czech authorities, in particular this :
- a certificate from a doctor or medical institution;
- subpoena;
- an invitation from a government agency;
- a certificate of the need for proper care (from a doctor or social service) and documents confirming family ties with this person;
- a court decision on the establishment of custody of the child or meetings with him.
Citizens of Ukraine who, in accordance with the above, have the right to enter the Czech Republic, must:
- before arriving in the Czech Republic, fill out the appropriate electronic form: https://plf.uzis.cz/. Filling out and sending the form automatically informs the relevant sanitary and epidemiological station about the person's arrival. Confirmation of filling out the form received by e-mail (printed or saved on a mobile device) must be submitted during border control or when checking the place of residence.
- when crossing the border with the Czech Republic, present a confirmation of a negative PCR test for coronavirus made no earlier than 72 hours before entry;
- not earlier than 5 days after arriving in the Czech Republic, do a second test for coronavirus in the Czech Republic;
- stay in self-isolation at home until the results of the second test are obtained;
- wear a respirator type FFP2 for 10 days after arriving outside your home.
