Since September 9, 2021, citizens of Ukraine have been added to the category of persons who are allowed to enter the territory of the Republic of Poland, including for tourism purposes - https://strazgraniczna.pl/pl/aktualnosci/informacje-dla-podrozny/8952,Koronawirus-wjazd-do-Polski-granica-zewnetrzna.html
For visa-free entry to Poland you need a valid biometric passport and get a health insurance policy for the entire period of stay in the country.
Citizens arriving from Ukraine must go through 10 days of self-isolation. The quarantine period will be reduced in case of a negative test result for SARS-Cov-2, performed no earlier than 7 days from the date of entry into the territory of Poland. List of laboratories - https://www.gov.pl/web/zdrowie/lista-laboratoriow-covid
The end of the quarantine is the presence in the teleinformation system of a record of a negative test result made by the diagnostic laboratory.
Tests for entering Poland from non-EU countries are not required.
Attention! The above also applies to children crossing the EU external border to enter Poland.
Persons are exempted from the obligation to stay in quarantine after crossing the external border of the EU:
- vaccinated against COVID-19 (if there is English / Polish documentary evidence of the full course of vaccination with a vaccine registered in the EU - Pfizer, Moderna, Astra-Zeneca, Johnson & Johnson). From the moment of the last dose of the vaccine, at least 14 days must pass (the day of vaccination is considered zero). Vaccination can be confirmed with a digital certificate generated in the Diia app, or printed from the Diia portal. Free entry also for children of vaccinated parents if they are less than 12 years old;
- recovered from COVID-19 (if there is documentary evidence);
- defined in § 3 of the Regulation of the Council of Ministers on the establishment of restrictions and prohibitions in connection with the epidemic (drivers and crew members of air, railway, shipping and motor vehicles, inspectors of the maritime administration, students + accompanying person, students, trainees, applicants for postgraduate education, doctoral students , diplomats, doctors, workers in agricultural enterprises, athletes + coaches, doctors, physiotherapists, judges, accredited journalists who enter Poland to participate in sports events, persons who have a permanent residence permit or a long-term resident card of an EU member state + members their families entering Poland for the purpose of transit through its territory).

From July 17, 2021, all persons arriving in the Republic of Poland must fill out the Traveler Location Card (eKLP) electronically before departure. The form can be found here: https://aplikacje.gov.pl/app/klp/#/home
