What to do if you have lost a residence card (karta czasowego pobytu): an algorithm of actions
There are many situations when you can lose or damage your residence card in Poland. We will tell you what to do in this case, where to go first and what the cost of replacing the document is
A residence card (karta czasowego pobytu) is a document confirming the identity and legal status of a foreigner in Poland. Since it looks like a plastic card with a photo of the holder, it is easy to lose. We will tell you what to do in case of loss of the document below.
Immediately contact the Uzhond
As soon as a foreigner notices that his or her karta czasowego pobytu is missing, he or she should contact the Uzhond.
The following deadlines must be met:
- Within three days, notify the Uzhond where you received the lost document;
- within fourteen days, submit an application for the restoration of the beat card.
The application can be submitted either in person or by mail by sending a letter with acknowledgment of receipt.
What documents are required to restore a karta czasowego pobytu?
In addition to the application for loss of the card, the following documents must be submitted:
- A completed card replacement request form;
- two photos of 35*45 mm in size;
- copies of all filled pages of the passport and the original document;
- copy and original of the "zaświadczenie o zameldowaniu". Please note that the document is issued by the Uzhond upon presentation of a passport, Pesel and a rental agreement. Issuance of this certificate costs PLN 17;
- a receipt of payment for the service.
If the institution reports that certain documents are missing, the applicant has seven days to provide them.
After submitting the application and documents, the office will issue a certificate confirming the foreigner's legal stay in Poland. It is valid until the day of receiving a new residence card, but not longer than two months.
The cost of replacing a residence card
There is a fee for reissuing a residence card. If a foreigner applies for such a card for the first time, the cost of the service is PLN 200. If the application is repeated, the price increases to PLN 300.
Some categories of citizens can receive a 50% discount. These include:
- Foreigners in a difficult financial situation;
- foreigners staying in Poland for the purpose of studying at school or college;
- children under the age of 16.
