You can now fill out a declaration with your doctor online: how do you do it?
A new digital service has been launched in Ukraine that changes the usual approach to choosing a family doctor and interacting with the healthcare system. Now, signing declarations, updating personal data, and controlling access to medical information are available online — without queues or visits to the clinic. A personal patient account has been launched in Ukraine, allowing users to remotely register with a doctor, update their personal data, and manage access to medical information without visiting a clinic. The launch of the service was announced by the National Health Service of Ukraine.
The account is available to all adult citizens, as well as to adolescents aged 14 to 18 with an electronic signature. Users can view their data and, if their legal capacity is confirmed, submit declarations and make changes to their personal information.
What features are already available
Through the patient account, you can view and update your personal data in the electronic health care system, see current and previous declarations with your doctor, find a doctor and fill out a declaration online, as well as manage access to electronic accounts that you have authorised to process your personal data. Access to medical records — tests, prescriptions, and diagnoses — will be added in stages as the service develops.

How to submit a declaration online
You can log in to your patient account through state or private electronic accounts. Their basic functions are the same, but additional features may vary. Users can choose a convenient service from the list of available options, which will gradually expand. After logging in, simply find a doctor, confirm your details, and sign the declaration with an electronic signature.
If you have any questions while using your account, you should first refer to the ‘Frequently Asked Questions’ section on the specialised portal, which contains answers to typical queries. In case of technical problems (e.g., unable to log in), users of the state account can create a request on the support portal by selecting the ‘Technical Issues’ category. If you are using another service, you should contact its support service. Issues related to personal data — correcting duplicate information or changing your RNUKP — can be resolved by contacting support and selecting the ‘Contextual Questions’ category.
Important to remember: to stay in any EU country, medical insurance with coverage of at least €30,000 is mandatory. You can purchase medical insurance for travel abroad online on the Visit Ukraine platform — this allows you to be protected in case of illness, injury, or unforeseen expenses while traveling.
Reminder! Ukraine is launching the ‘Health Screening 40+’ programme, which allows you to check your health for free without queues or referrals. Invitations will be sent to the ‘Dія’ app after your birthday, and the funds will be automatically credited to your ‘Dія.Card’.
