Health insurance is mandatory for all residents of Germany. Most employees use the state insurance system - Gesetzliche Krankenversicherung (GKV). One of its important advantages is the ability to include family members in one policy free of charge. This model is called Familienversicherung.
Thanks to family insurance, spouses and children can receive medical services without paying additional contributions if they meet certain conditions. We tell you who is entitled to Familienversicherung, what age restrictions exist and how to arrange such insurance.

What is Familienversicherung?
Familienversicherung is a form of family health insurance within the German state healthcare system. It allows you to insure family members without additional contributions if the main insured person is already a member of the state insurance system.
In this case, only one person pays the premiums – for example, an employee – and their family has the same access to healthcare as the employee. Familienversicherung is one of the key social features of the German healthcare system, which helps to reduce the financial burden on families.
Under the terms of Familienversicherung, all included family members receive the same amount of healthcare as the main insured person.

Who can be insured through family insurance in Germany?
The following family members can be added to the policy:
- Spouse or registered partner, if they do not have their own compulsory health insurance, are unemployed or have a low income.
- Children (relatives, adopted children, stepchildren, foster children, sometimes grandchildren, if they are dependent).
Are there age limits for free family insurance?
The term of free insurance for children depends on their age and status:
- up to 18 years – without additional conditions;
- up to 23 years – if the child is unemployed;
- up to 25 years – if they are in school, undergoing vocational training or studying at university.
In some cases, this term can be extended, for example, if their studies were interrupted due to military service or volunteer programs.
For children with disabilities, family insurance can operate without age restrictions if they are unable to provide for themselves.
When can a child not be included in family insurance?
In some cases, a child cannot be included in Familienversicherung. For example, if one of the parents has private health insurance and his or her income exceeds the income of the father or mother with state insurance. In such a situation, the child will have to take out separate insurance.

What conditions must be met for Familienversicherung?
In order for a family member to be included in the family insurance, the following conditions must be met:
- The person must reside permanently in Germany.
- They must not have their own compulsory health insurance.
- They must not be fully self-employed.
- Their regular income must not exceed a certain limit.
In 2026, the income threshold is approximately 565 euros per month. For so-called part-time employment (Minijob), the permissible income can be up to 603 euros per month.

What income is taken into account when receiving Familienversicherung?
When checking the right to Familienversicherung, a person's total income is taken into account. This may include: salary, self-employment income, rental income, income from investments or capital, pensions and other regular payments.
If the total income exceeds a certain limit, free family insurance no longer applies, and the person must take out their own health insurance.
How to take out family insurance in Germany?
To include family members in Familienversicherung, you need to contact your insurance company and submit an appropriate application. Typically, the insurance company may request the following documents:
- Marriage certificate.
- Birth certificate of children.
- Information on the income of family members.
- Proof of residence.
After checking the data, the insurance company makes a decision on including family members in the policy.
Can Ukrainians use Familienversicherung?
Ukrainians who live and work in Germany and have state health insurance can also benefit from Familienversicherung. If their family members live in Germany and meet the income and employment requirements, they can be included in the family insurance without additional contributions.
Family insurance in the Gesetzliche Krankenversicherung system allows you to significantly reduce the cost of medical services for families. With Familienversicherung, spouses and children can receive full medical coverage without separate insurance contributions if they meet the income, residence and employment requirements.
